Unity Health Network formed as a group of physicians who identified the benefits of multi-specialty collaboration.
The organization’s focus has always been to provide the best possible care to patients, while making access to providers a top priority. This increased level of access to care allows Unity Health Network to achieve high levels of patient satisfaction, while leading the region in a value-based care approach. Since its inception in 2012 the organization has grown to almost 100 providers, in 4 counties and caring for nearly 200,000 patients annually. Provider Clinical Independence
Unity Health Network is a physician-led independent multi-specialty group in Northeast Ohio. Our organization allows for autonomy of the Network members, while providing the benefit of shared operational savings through efficiencies of scale.
Comprehensive Operational Infrastructure
Resources integral to your practice success include our teams responsible for billing, revenue cycle management, human resources, credentialing and marketing. Additional support is also provided for management of information technology, electronic medical record and comprehensive data reporting.
Quality
As an organization focused on quality and controlling costs we created a multi-level infrastructure that manages the daily operational functions vital to the growth and success of a medical practice. Our Care-Coordination center is an all-in-one access point for patient scheduling and information needs. It also serves as our center for all care management and population health oversight including: case management of high risk patients, chronic care management, telemedicine and digital remote monitoring of patients. Our state of the art electronic medical record provides us with real time data for improving patient care and increasing productivity.
Moving Forward
With expansion and growth our collaboration stretched outside of practice development. In 2015 we became a founding member of the Ohio Independent Collaborative (OIC). A statewide consortium of nearly 1000 independent physicians that work together to improve quality, patient care and access. With this partnership we are able to continue with our original goal, provide high quality care at a lower cost and expand our reach across Northern Ohio while maintaining the most important of all things, patient centered care.
